บทคัดย่อภาษาไทย

 การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษากระบวนการจัดการสุขภาพชุมชนและศึกษาแนวทางกาพัฒนากระบวนการจัดการสุขภาพชุมชนของโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบ่อทราย อำเภอป่าพะยอม จังหวัดพัทลุง เป็นการวิจัยเชิงคุณภาพ เก็บรวบรวมข้อมูลโดยการสัมภาษณ์จาก กลุ่มผู้ให้ข้อมูลหลัก จำนวน 26 คน กำหนดกลุ่มผู้ให้ข้อมูลแบบเจาะจง จำนวน 20 คน ซึ่งประกอบด้วย ผู้อำนวยการโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบ่อทราย อาสาสมัครสาธารณสุข (อสม.) ผู้นำชุมชน พระภิกษุ ครู กลุ่มชมรมผู้สูงอายุ และเลือกกลุ่มผู้ให้ข้อมูลแบบบังเอิญ จำนวน 6 คน ซึ่งเป็นประชาชนผู้มารับบริการที่ รพ.สต.บ้านบ่อทราย อำเภอป่าพะยอม จังหวัดพัทลุง เครื่องมือที่ใช้ในการวิจัย คือ แบบสัมภาษณ์กึ่งโครงสร้าง การบันทึกภาพและบันทึกเสียง                                                                     

ผลการวิจัยพบว่า 

กระบวนการจัดการสุขภาพชุมชนของ โรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบ่อทราย อำเภอป่าพะยอม จังหวัดพัทลุง มีขั้นตอนการดำเนินงาน คือ 1) การสำรวจข้อมูล สำรวจข้อมูลด้านสุขภาพ ปัญหา และความต้องการของคนในชุมชน การสร้างทีมแกนนำซึ่งเป็นทุนทางสังคมที่สำคัญของชุมชน การเปิดโอกาสการเข้าร่วมแบบจิตอาสา การสร้างข้อตกลงร่วมในการกำหนดบทบาทหน้าที่และความรับผิดชอบอย่างชัดเจน การออกแบบกิจกรรมการจัดการสุขภาพ  ภายใต้แนวคิดการพัฒนาศักยภาพการจัดการตนเองประชาชน 2) ผ่านกระบวนการมีส่วนร่วมในเวทีประชาคม และการประชุมร่วมกันเพื่อแลกเปลี่ยนเรียนรู้ เป็นการคืนข้อมูลกลับสู่ชุมชน 3) มีการกำหนดเป้าหมายร่วมกัน วางแผนและดำเนินงานร่วมกันอย่างมีทิศทาง และเข้ามามีส่วนร่วมในกิจกรรมต่าง ๆ ทั้งในระดับบุคคล และระดับกลุ่ม รวมทั้งผลักดันโครงการเข้าสู่แผนพัฒนาชุมชนทีมีความสอดคล้องกับนโยบายบริหารงานในระดับท้องถิ่น 4) และร่วมกันปฏิบัติตามแผนที่ได้จัดทำขึ้น 5) มีการประเมินผลจากกิจกรรมและโครงการต่าง ๆ 6) ร่วมกันสรุปถอดบทเรียน ในส่วนที่ยังบกพร่อง ก็นำมาปรับปรุงแก้ไข โครงการหรือกิจกรรมที่ประสบความสำเร็จก็มีการขยายผล และเป็นต้นแบบให้กับรพ.สต.อื่น ๆ ในการจัดการสุขภาพชุมชนอย่างมีประสิทธิภาพต่อไป                                                          

ส่วนแนวทางในการพัฒนากระบวนการจัดการสุขภาพชุมชนของโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบ่อทราย อำเภอป่าพะยอม จังหวัดพัทลุง พบว่า โรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบ่อทราย ควรมีการสร้างภาคีเครือข่าย สร้างแกนนำ และให้มีการรวมกลุ่มของคนในชุมชนเพื่อทำให้การจัดการสุขภาพในชุมชนมีความเข้มแข็งโดยการบริหารจัดการร่วมกันโดยคนในชุมชนด้วยกันเองให้มากขึ้น มีการเสนอของบประมานจากหน่วยงานที่เกี่ยวข้องเพื่อให้โครงการที่กำหนดขึ้นสามารถนำไปใช้ปฏิบัติได้จริง ควรมีการร่วมประเมินผลจากหน่วยงานอื่น และ ให้มีการประสานกับภาคีเครือข่าย ร่วมกันสรุปถอดบทเรียนและเผยแพร่ผ่านสื่อต่าง ๆ เช่น เว็บไซต์ของ รพ.สต. หรือวิธีการอื่น ๆที่ง่ายต่อการเข้าถึงข้อมูลของประชาชนทุกกลุ่มเพื่อที่จะได้สามารถนำผลสำเร็จจากโครงการที่ได้จัดทำขึ้นไปประยุกต์ใช้เป็นแบบอย่างในการจัดการสุขภาพชุมชน ที่มีประสิทธิภาพและยั่งยืนต่อไป 

บทคัดย่อภาษาอังกฤษ

The objectives of this research were : to study on procedure of community health development and to study on guideline for community health development of Banborsai sub-district health promoting hospital, Papayom district, Patthalung province.  This is qualitative research. Data collection from interview of key informants for 26 persons, sampling by purposive for 20 persons which are composed of head of Banborsai sub-district health promoting hospital, public health volunteers, community leaders, monks, teachers, aging group association and sampling by accidental for 6 persons which are composed of people who came as recipients of Banborsai sub-district health promoting hospital, Papayom district, Patthalung province.  The instrument for this research was sami-structure interview form, tape record sound and photo.                                                                           

The findings were as follows.: 

The procedure of community health development, Banborsai sub-district health promoting hospital, Papayom district, Patthalung province.  The process can be summarized as follows ; 1) to survey of data in the aspect of health, problem and community’s need, the set of leading team which is the main social capital in community, to give an opportunity to free participation, to set agreement in the role and function and responsibility as clearest, the arrangement of health activity management under concept of competency development, self-management of people passed throughout community forum participation. 2) Co-operation meeting for changing knowledge, as a feedback of information to community, to have a co-operate good throughout planning. 3) Processing as focus on directing and to participate in any activities righter personal or group level and to lift up project into community development plan which are accordance with local administration policy.4) to participate according to setting plan. 5) to evaluate from activities and projects and       6) joined to summarize the lesson, for the defective path, it must be improved and for the successful activities and projects must be extended and to a good idol to offers hospital in community health promoting management as effectively in the future.

And the guideline for process development of community health promoting management  of Banbosai sup-district hospital , Papayom district, Patthalung province found Banbosai sup-district hospital should have networking, leader group, and to collect a group of people in community for community health management as strongly with co-operation administration by people in community  as more and more, to request for budget from concerning units for the project can be work property, there should have others units participating for evaluation and to be co-operate with networking for participating in summarizing of lesson and publishing throughout multi medias i.e. website of sub-district health promoting hospital or any others which are easy to searching of information of any people groups for beginning the success of project that arranged to apply as the model in community health management as effectively and sustainability in the future.
